Terms and Conditions

I agree to become a member of Rapid Fitness, and agree to the following terms and conditions:

PAYMENT OF MEMBERSHIP DUES.
I agree to pay monthly membership dues, plus applicable taxes, each month thereafter through automatic withdrawals from an account maintained in a financial institution. Additionally, the member shall pay this agreement, an initiation fee of Ten Dollars($30), unless otherwise stated(redeeming a special).

I agree to pay membership dues plus applicable taxes on the monthly first, every month until the membership is terminated(refer to the cancellation policy).

If I signed up for a 12-month agreement, I agree to pay at least 12 monthly payments. At the end of the 12-Month Agreement, the contract will continue on a month-to-month basis. If I terminate a 12-Month agreement before 12 Monthly Payments have been collected, the Member agrees to pay $15 for every month their Membership has been paid for.

CANCELLATION POLICY. Automatic drafts will not be cancelled without fifteen(15) days written notice, which must be filled out at the Rapid Fitness front desk.

RELEASE AND WAIVER OF LIABILITY.
I/We hereby understand and acknowledge that the training, programs and events held by Rapid Fitness may expose me to many inherent risks, including accidents, injury, illness, or even death. I hold Rapid Fitness harmless and release them from liability.


SURVEILLANCE/UNAUTHORIZED ACCESS.
I acknowledge that surveillance cameras are in use for the protection of the facility, its equipment and members. If I allow unauthorized access I agree to pay a fee of $20 per person per offense.

ACCEPTANCE OF TERMS. As a Member, I understand that I am entitled to use the Facility within the scope of the membership I have selected, and I am obligated to pay my dues and fees regardless of whether or not I use the Facility.

I certify that I have read the foregoing Membership Agreement, and by agreeing to the terms and conditions, I acknowledge that I understand and agree to be bound by all of the terms and conditions hereof. I further acknowledge that a fully executed copy of this Membership Agreement has been provided to me.
